Instructions for Applying Electronically
You may want to print this page for your
convenience as it is very important that your payment and your e-mail are
received almost simultaneously. We receive 100's of applications so the
closer together they are in our list, the easier it will be to identify and
process your application.

Select an electronic payment option from the Make Payment page.
PayPal is strongly recommended as it is very secure and your payment will be
processed immediately. For your convenience we also accept Visa, Master
Card, and American Express. Don't worry, If your application is not
accepted, your payment will be refunded to you.

Please be sure to enter the ID# WA98271 on your payment form in the
proper field. Also, it is very important that you use the same name and
e-mail address that is on your application. Both application and payment
should match exactly. This is the only way we can ensure that your
payment is applied to your application.

are mandatory. Please ensure that the ID # WA98271 appears on the
application.

the submit button

Instructions for Applying using US Postal Service
1)
Print the application form and fill in all the required information. Please
print or type clearly.
2)
Enclose Check or Money order payable to Thomas Farber.
3)
Ensure that you have included the ID# WA98271 on both documents.
4)
Mail Application and Payment to:
